Assessment Report Templates

All undergraduate academic programs at Montana State University are required to submit a program assessment report annually for undergraduate programs, and biennially for graduate programs. See Graduate Assessment Cycle below.

Programs that are externally accredited do not need to submit an annual or biennial MSU assessment report, but should still be collecting appropriate data for their external accreditors as well as for their 7-year Program Reviews.

Annual, Biennial, and Year 0 Assessment Planning Reports are due on October 15th every year.

The templates below are designed to guide you in the program assessment process and include detailed instructions.  The Assessment and Outcomes Committee (AOC) will provide feedback on the assessment report based on best practices using a specially-designed Rubric.
