Keeping records is a valuable life skill.  The type of form used is not as important as the skill learned.  The 4-H software, ZSuite, has a record book option.  Please log in to your ZSuite account to get started on your record books.

The Montana 4-H program created forms in 2016 for 4-H members to use.  Those forms are below.  They can be printed and handwritten, or downloaded as an electronic file and updated using a computer. 

Each member should have:

  • One My 4-H Year Form
  • A Project Journal for each project they are taking that is a non-animal project.
  • An Animal Project Journal for each animal project they are taking.

They can be saved to your computer for electronic use throughout the year
You can type your record book information right into them.
YOU MUST save each file to your computer to use form functions. 

If you choose to save any of the files for electronic use,
you must have version 10 or higher of Adobe Reader.
